APPLICANT REQUESTS THAT:

His DD Form 214, Armed Forces of the United States Report of 
Transfer of Discharge, be amended to reflect all awards earned, 
and to correct the following errors:

1.  Add the Jungle and Mountain training courses.

2.  Add the Air Force Commendation Medal (AFCM).

3.  His social security number is incorrect.


APPLICANT CONTENDS THAT:

The last two digits of his social security number are transposed 
on his DD from 214, as evidenced by the social security card 
submitted with his request.  Also, he completed the Jungle and 
Mountain training courses and they are not noted in his record.  
Further, he was awarded the AFCM from his assignment in Thailand 
and it was omitted from his record.

Exhibit A.


STATEMENT OF FACTS:

On 28 Sep 64, the applicant entered the Regular Air Force.

According to the applicant’s AF Form 7, Airman Military Record, 
his social security number is not the same number as reflected his 
DD Form 214, but is the same number as reflected on the social 
security card submitted with his request.

From 19 Jul 67 to 18 Jul 68, according to the applicant’s 
AF Form 7, he served in Thailand.

On 18 Jul 68, the applicant was furnished an honorable discharge, 
and was credited with 3 years, 9 months, and 21 days of active 
service, which includes 1 year of foreign service.

The remaining relevant facts pertaining to this application are 
contained in the memoranda prepared by the Air Force offices of 
primary responsibility (OPR), which are attached at Exhibits C, D, 
and E.


AIR FORCE EVALUATION:

AFPC/DPSIRP recommends to grant relief to correct the applicant’s 
social security number, indicating there is evidence of an error 
or an injustice.  A review of his personnel records revealed 
multiple source documents to validate his social security number 
is incorrect on his DD Form 214 and should be corrected.

A complete copy of the AFPC/DPSIRP evaluation is at Exhibit C.

AFPC/DPSIT recommends denial to add the Jungle and Mountain 
training courses to his DD Form 214, indicating there is no 
evidence of an error or an injustice.  Upon review of applicant’s 
record, no evidence was discovered to validate he completed the 
Jungle and Mountain training courses.  Without proof of training, 
such as a certificate of completion, no correction to the 
applicant’s DD Form 214 is recommended.

AFPC/DPSID recommends denial of the applicant’s request to award 
the AFCM, indicating there is no evidence of an error or an 
injustice.  The AFCM is awarded to members of the Armed Forces of 
the United States, below the grade of colonel and foreign 
military personnel, who, while serving in any capacity with the 
Department of the Air Force, after 24 Mar 58, distinguish 
themselves by outstanding achievement or meritorious service.  
No official documentation (such as the Special Order) was located 
in the applicant’s record, nor was any provided with his request 
to verify he was awarded or recommended for the AFCM.  The Special 
Order is the official source documentation for verification of an 
authorized decoration.  In order to reasonably consider the 
applicant's request, he needs to submit a recommendation from 
someone with firsthand knowledge of the act/achievement preferably 
from someone within his chain of command at the time of the 
act/achievement, a proposed citation, and eyewitness statements.  
To grant relief would be contrary to the criteria established by 
DoDM 1348.33, Secretary of the Air Force, Chief of Staff, and/or 
the War Department.

Additionally, based on the documentation contained in the 
applicant’s record, it was verified the applicant meets the 
eligibility criteria for the Air Force Outstanding Unit Award with 
three Bronze Oak Leaf Clusters (AFOUA w/3BOLC), the Vietnam 
Service Medal with three Bronze Service Stars (VSM w/3BSS), and 
the Republic of Vietnam Gallantry Cross with Palm (RVNGC w/P).

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluations were forwarded to the 
applicant on 23 Jan 15 for review and comment within 30 days 
(Exhibit F).  As of this date, no response has been received by 
this office.


THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing 
law or regulations.

2.  The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file.

3.  Insufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice.  We took 
notice of the applicant’s complete submission in judging the 
merits of the case; however, we agree with the opinion and 
recommendation of the Air Force office of primary responsibility 
(OPR) and adopt its rationale as the basis for our conclusion the 
applicant has not been the victim of an error of injustice.  
Therefore, in the absence of evidence to the contrary, we find no 
basis to recommend granting the requested relief.

We note the OPR has determined the applicant’s eligibility for the 
AFOUA w/3BOLC, the VSM w/3BSS, and the RVNGC w/P and therefore 
will correct his records administratively.  Therefore, in the 
absence of evidence to the contrary, we find no basis to recommend 
granting relief beyond that rendered administratively.

With regards to the request to correct the applicant’s social 
security number, sufficient relevant evidence has been presented 
to demonstrate the existence of an error or injustice.  We took 
notice of the applicant's complete submission in judging the 
merits of the case and agree with the opinion and recommendation 
of the Air Force office of primary responsibility (OPR) and adopt 
its rationale as the basis for our conclusion the applicant has 
been the victim of an error or injustice.  Therefore, we recommend 
the applicant's records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force 
relating to the APPLICANT be corrected to show that the 
DD Form 214, Armed Forces of the United States Report of Transfer 
or Discharge, be corrected to reflect his social security number 
as xxx-xx-xxx.
